I have found two different kinds of custom boot-shoe design. The first is really semi-custom. It is where you get on a website and choose the material (suede, leather, lizard etc) then choose the heel, the height of the shaft, the circumference of shaft, etc. Usually from a drop down menu. This allows you to get what you want, but you are limited to what the site has to offer. It keeps the cost of the boot down. They don't really have to "make" anything new.
The second custom boot option is truly custom. They take about 7 or 8 different measurements of your foot. Your leather is usually on the shelf. Then you choose from a menu of toe shapes, from heel shapes, heel height, and so on. They are more expensive because this boot is made for you. I have one pair of custom made boots. They aren't anything sexy, - chocolate brown, bullhide, flat heel, round toe- but if I am going to be out and about, in boot weather, these are the ones I put on because they fit like a dream. I want another pair.
To answer the question...How are these boots constructed. Will the sole be leather? or Manmade? Will they be sewn on or glued? What kind of leather is offered? I have paid an obscene amount for a pair of custom boots for DH, but he has worn them for 20 years.
